Terms and Conditions of Free Companion Flight Offer:

1. This offer is available between November 19, 2008 and January 5, 2009 (“Offer Window”), or while supplies last. This offer entitles an eligible T-Mobile accountholder to receive one free round-trip companion airline ticket with the purchase of a full zone fare ticket, up to a maximum of five (5) Free Companion Flights per account, subject to the following terms and conditions.
2. In order to qualify for this offer, you must have either: (a) activated a new eligible line of service with T-Mobile, (b) renewed your existing line of service, or (c) added an eligible line of service with a two-year service agreement during the Offer Window.
3. Lines of service under the following account types are ineligible for this offer: T-Mobile business/government (non-individual liable), Prepaid, special/employee, and Business Direct channel.
4. You must remain a customer in good standing for at least 31 days from the date of activation, renewal or adding a line before you are eligible to book your Free Companion Flight.
5. Eligibility is limited to legal residents of the 50 United States, Washington, D.C., and Puerto Rico.
6. In order to redeem the Free Companion Flight offer, you must first register online at www.flywitht-mobile.com (“Site”) with your full name, e-mail address, T-Mobile account number, and T-Mobile wireless number.
7. Once you are validated as an eligible T-Mobile customer that meets the qualifications for this offer, you will receive an e-mail with a unique personal identification number (PIN) that will allow you to purchase your full coach fare ticket and book your Free Companion Flight through the online booking engine at the Site. If for any reason you are not validated, you will be notified by e-mail explaining the reason for the disqualification.

9. The Free Companion Flight offer is valid for standard travel in coach class with travel dates commencing and concluding on or before February 15, 2010.
10. Free Companion Flights must be confirmed for the same dates and flight itinerary as the tickets purchased at regular price.
11. Open tickets, open returns, open jaws and circle trips are not permitted and stopovers are not allowed.
12. Free Companion Flights may be redeemed for travel between hundreds of U.S. destinations. Additionally, customers may originate their travel in San Juan, PR or Honolulu, HI. These two cities may not be selected as destinations. The list of available cities may be subject to change without notice.
13. The selected departure and destination cities will determine the airline carrier used, at the sole discretion of Caliber.
14. A booking service fee will apply to each flight booked, including the flight(s) purchased at regular price and the Free Companion Flight(s). If tickets are booked through the Site, the booking service fee is $12.75 per ticket. If tickets are booked over the telephone with a live agent using Caliber's Concierge Service, the booking service fee is $30.00 per ticket.
15. You are responsible for the payment of all applicable taxes, fees and surcharges imposed by the airline, airport, and/or government including, but not limited to optional travel insurance, fuel surcharges, security fees, baggage and luggage fees, premium seat selection fees, food and beverage costs, unaccompanied minors, and entertainment charges.
16. Additional passengers may be booked for the applicable standard airfares, and all taxes and charges as outlined in Points 16 and 17 shall apply.
17. No cancellations or itinerary changes are possible through Caliber after confirmation of booking. Upon confirmation, all changes/cancellations must be made with the airline and such airline’s cancellation/change fees and policies may apply.
18. This offer cannot be used in conjunction with any other discount or promotional offer.
19. T-Mobile and Caliber are not responsible for any alterations to flight schedules, routes or destinations and reserve the right to use alternative suppliers. We reserve the right to withdraw this offer without notice in the event of war, earthquake, catastrophe, strikes or similar event.
20. By completing the online registration and using the online booking engine at the Site or the Travel Concierge, and/or traveling pursuant to this offer, you agree to release, discharge and hold harmless Caliber Agency, T-Mobile USA, Inc., their respective parents, affiliates, subsidiaries, retailers, agencies and their respective officers, directors, employees and agents, from any and all liability, claims, judgments, losses, injuries, demands, damages, actions, and/or causes of actions, whether direct or indirect, arising from or relating to participation in this promotion, flights, or travel accommodations.
21. Free Companion Flights can be transferred or given as a gift by sharing the booking PIN. Free Companion Flight cannot be sold, resold, returned, bartered, auctioned or redeemed for profit. There is no cash alternative to this offer. Free Companion Flights have no monetary value. Free Companion Flights shall be deemed void where prohibited or restricted by law.
22. Employees or agents of T-Mobile, its affiliates, subsidiaries or vendors specifically involved in this promotion, are not eligible. Employees of Caliber or any of their group companies or any of the participating airlines or anyone professionally connected to this promotion are not eligible.
